Immunization policies for CEVSD

All students attending Carrollton Exempted Village Schools must be in compliance with state immunization requirements. This is accomplished in one of four ways:

— Have on file a record of having received all the minimum required vaccines. — In process. — Have on file a physician’s statement of medical inadvisability. This must be updated yearly. — Have on file a statement of philosophical or religious objection signed by the parent or guardian.
