Nurse practitioner Simona Moore leads a new era of family-focused, accessible primary care downtown

Residents of Mount Vernon and surrounding communities now have a new healthcare home with the grand reopening of Moore Family Practice LLC on Sept. 3, located at 206 Mulberry Street, Suite A, Mount Vernon.
The practice, led by founder and board-certified Nurse Practitioner Simona L. Moore, RN, MSN, CNP, officially reopened its doors last week, promising comprehensive, patient-centered primary care for individuals from infancy through the elderly.
Originally established in 2013 as Simona L. Moore CNP LLC, the practice underwent a rebranding in 2019, becoming Moore Family Practice LLC to better reflect its commitment to providing family-focused care across all ages. This evolution highlights the growth and deepening connection with the Mount Vernon community over the years.
Moore provides a wide range of primary care services designed to support patients at every stage of life. These services include routine health screenings, management of chronic diseases such as diabetes and hypertension, sick visits for acute illnesses, preventive care, and health education. While Moore Family Practice delivers these core services, vaccines and laboratory testing are completed in collaboration with the Knox County Health Department to ensure patients receive thorough and streamlined support.
The practice also provides referrals to medical specialists when needed. Moore Family Practice accepts most insurance plans, including Medicaid, making quality healthcare accessible and affordable for a wide range of patients.
Moore brings more than 25 years of direct patient care experience to her role as founder of Moore Family Practice. She holds a Family Nurse Practitioner certification from Otterbein University, a Master of Science in Nursing (Adult Nurse Practitioner) from The Ohio State University, and a Bachelor of Science in Nursing.
Her professional background includes serving as a Certified Nurse Practitioner at Knox Public Health, where she provided both acute and primary care across all age groups. Throughout her career, she has also worked in private practice, walk-in clinics, and hospital-based settings. In addition to clinical work, Moore has served as adjunct clinical faculty at Otterbein University, precepting nursing and nurse practitioner students in hands-on primary care training.
Known for her evidence-based approach and compassionate care, Moore has developed programs focused on diabetes education, chronic disease management, and anticoagulation therapy. Her leadership in both clinical practice and patient education underscores her dedication to improving health outcomes in her community.

Moore Family Practice shares its office space with Simona Moore’s son, Jordan Moore, APRN, PMHNP-BC, a U.S. Army veteran and dual board-certified nurse practitioner. Jordan is the founder of Revive Behavioral Health, a new mental health and addiction recovery service provider in the community that opened for business on Aug. 4. This collaboration creates a unique healthcare environment where physical and behavioral health services coexist, providing a more holistic approach to patient well-being.

• Monday: 7:30 a.m. to 5 p.m.
• Tuesday: 7:30 a.m. to 3 p.m.
• Wednesday: 7:30 a.m. to 1 p.m.
• Thursday: 7:30 a.m. to 4:30 p.m.
• Friday: 9-11:30 a.m. (primarily reserved for telehealth visits)
