Revive Behavioral Health opens in Mount Vernon with veteran-led mission
New downtown clinic provides psychiatric care, addiction recovery, and innovative treatments like TMS therapy

A powerful new resource for mental health and addiction recovery has opened its doors in the heart of Mount Vernon.
Revive Behavioral Health, located at 206 S. Mulberry St., Suite A, officially began serving patients on Aug. 4, providing a full range of mental health services, psychiatric care, and addiction treatment options. Founded by Jordan Moore, APRN, PMHNP-BC, a U.S. Army veteran and dual board-certified nurse practitioner, Revive is quickly becoming a cornerstone of care for the Knox County community.
Moore’s career spans both military and medical service. After serving eight years as a Military Police Staff Sergeant, completing two combat deployments, and later commanding a 32-bed field hospital, he transitioned into healthcare leadership, ultimately earning board certifications as a Family Nurse Practitioner and Psychiatric Mental Health Nurse Practitioner. He left the military as a Captain in May 2024.
When asked what motivated him to open his own practice, Moore was clear: “The need.” Locating Revive in downtown Mount Vernon was intentional — to ensure accessibility for individuals who may rely on walking or public transportation to get to appointments.
“I’m especially passionate about servicing veterans,” Moore added, reflecting on his military background. While Revive is deeply committed to serving veterans, its services are designed to support the entire community. The clinic welcomes individuals from all walks of life — parents, teens, professionals, and anyone ready to begin the healing process.

Deborah Wauthier, MSW, LSW, LICDC brings expertise in addiction recovery and trauma-informed therapy, while Jacqueline Pack, MSW, LSW focuses on supporting clients dealing with depression, anxiety, and life transitions. Revive is committed to making mental health care accessible by offering flexible hours that accommodate busy schedules:
• Monday: 7 a.m. to 7 p.m.
• Tuesday: 7 a.m. to 3:30 p.m.
• Wednesday and Thursday: 7 a.m. to 4:30 p.m.
• Friday: 7 a.m. to 2 p.m. (Fridays primarily offer telehealth appointments)
“We are open when nobody else is,” Moore said, highlighting the clinic’s focus on availability when other providers may be closed.
Revive accepts most major insurance plans, including Medicaid, further reinforcing its commitment to affordable, accessible care for all members of the community.
While there may be plans for future expansion outside of Knox County, Moore says the immediate focus is on the people of the Mount Vernon community.
